Disciplinary History

W&J strives to maintain a safe, inclusive, and tolerant campus. While having a disciplinary or criminal record will not necessarily preclude your eligibility for admission, we retain the right to work with applicants toward the goal of a safe and inclusive campus. Accurate disclosure of disciplinary or criminal history offers applicants an opportunity to explain their circumstances and allows the university an opportunity to make informed admissions decisions. The vast majority of applicants who answer “yes” to these questions will still be eligible to be considered for admission.

If you answer “yes” to any of the three questions below, please provide an explanation that gives the approximate date of each incident, explains the circumstances, and reflects on what you learned from the experience.

If you answer “yes” to any of the three questions below, no action will take place on your application until your explanation has been received and reviewed.

Have you ever been found responsible for a disciplinary violation at any educational institution you have attended from the 9th grade (or the international equivalent) until now, whether related to academic or behavioral misconduct that resulted in a disciplinary action? These actions could include but are not limited to probation, suspension, removal, dismissal, or expulsion from the institution.

Have you ever had a protective, restraining, or stalking order entered against you?

Contact Information

List parents or legal guardians who know where you can be reached. Other relatives or friends may be listed.

Do you have potential eligibility for any services or benefits related to military or dependent status?

While federal guidelines determine the way we officially report on students on our campus in regard to ethnicity and race, some students find the official ques- tion to not be a good fit for how they define themselves. This question, while not used in our official reporting, provides these students with a chance to say more about themselves, and for the W&J to learn more about our student body in the process. Your answer will not be used in our admission decision.

Regardless of how you answered our federal question regarding race and ethnicity, do you consider yourself to be multiracial/multiethnic?
